Super contributions often require specific forms to be lodged with your fund or the ATO. This guide lists the key forms, explains when you need them, and provides direct links to download the latest versions.

Important: Deadlines and eligibility rules matter. Lodging late (or after a trigger event) can change how a contribution is treated and may create excess contributions or missed tax outcomes.
– Notice of Intent to Claim a Deduction
📌 What it’s for
Notifies your super fund that you intend to claim a tax deduction for personal contributions you have made. Without this notice and acknowledgement, your contribution is generally treated as non-concessional (no deduction).
⏰ When to lodge
You must lodge this notice before the earliest of:
- Lodging your tax return for that income year
- The end of the income year following the contribution year
- Rolling over or withdrawing the contribution
- Commencing a pension using those funds
📝 How to complete
- Download the form from the ATO website
- Complete your personal details (name, TFN, DOB)
- Enter the financial year of the contribution
- Enter the amount you intend to claim (can be less than total contributed)
- Sign and date the form
- Submit to your super fund (portal/email/post)
- Keep the acknowledgement letter from your fund

- You can vary your notice later using the same form (provided you haven’t triggered a deadline event).
- Many funds allow online lodgement through their member portal.
- Keep the fund’s acknowledgement for your records and tax return support.
NAT 75073 – Downsizer Contribution
📌 What it’s for
Elects to treat an eligible contribution as a downsizer contribution from the sale of your home. This helps your fund classify it correctly (it doesn’t count toward your NCC cap).
⏰ When to lodge
Provide this form to your super fund before or at the same time as making the contribution. If you lodge after, the fund may need to treat it as a normal NCC.
Contribution timing: generally within 90 days of receiving settlement proceeds (unless an extension applies).

- Download the form from the ATO website
- Complete your personal details
- Enter property details (address, contract/settlement date)
- Enter your contribution amount (max $300,000 per person)
- Declare you meet eligibility criteria
- Submit to your super fund with/before the contribution
✅ Eligibility reminder
- You must be 55 or older at the time of contribution
- The home was owned for 10+ years
- The home qualifies (or has qualified) for the main residence CGT exemption
- You haven’t previously made a downsizer contribution
NAT 15237 – Superannuation Contributions Splitting Application
📌 What it’s for
Requests your super fund to split (transfer) up to of your concessional contributions from the previous financial year to your spouse’s super account.
⏰ When to lodge
Generally lodge in the year following the contributions you want to split. Same‑year splitting may be available if you’re:
- Leaving employment with that employer, or
- Rolling over your entire balance to another fund.
📝 How to complete
- Download the form from the ATO website
- Complete your details and your spouse’s details
- Specify the financial year of contributions to split
- Enter the amount/percentage to split (max )
- Provide your spouse’s fund details
- Both you and your spouse sign
- Submit to your super fund
✅ Spouse eligibility (general)
- Under 65, or
- Aged 60–64 and not permanently retired
NAT 71162 – Contributions for Personal Injury
📌 What it’s for
Elects to make a cap‑exempt contribution from an eligible personal injury settlement or payment. Without the correct election and documentation, the contribution may be treated as a normal NCC.
⏰ When to lodge
Provide to your super fund before or at the same time as making the contribution.
